Sports Injury Claims Procedure
Players injured in the summer 2000/01 will still need to
use Jardine insurers and call 1300

Injuires sustainedİin competitions which commenced after
January 2001 should be claimed through Sportscover - Information
below.
1. When injured, report your injury to Administration immediately.
2. Request a Claim Form from Sportscover within 30 days
3. Complete and return the claim form to Sportscover within 90 days. Ensure your team, Elwood Park Touch Association and Victorian Touch Association are filled in. Please remember to complete all sections to avoid delay.
4. Get your medical practitioner to fill in the appropriate statement.
5. Claim from your Private Health insurer / Medicare, if applicable.
6. When Treatment is complete please return all Accounts to Sportscover.
Sports Injury Benefits (Non Medicare Medical)
The percentage of the expenses covered under this section is 75%.
The percentage of physiotherapy expenses covered under this section is 75%
The excess payable for each claim under this section is $ 100 Excess - Nil excess if claiming on your Private Health Insurance
The maximum amount payable per claim under this section is $ 2000
Benefit period is 12 months from the date of the injury
Please note: After six visits
to an ancillary provider a referral from a legally qualified
medical practitioner must be obtained.
Optional: Loss of Income
Loss of Income optional policies may be taken out to cover full time employed participant's.
Insurance can be effected on an Association basis.
Student Benefits
Pays up to 75% of non medical expenses to a limit of $1500 being costs incurred for tutoring, travelling costs, etc. (on Doctor's authority) to assist the full time student
Excess: 14 days per claim
Cover is for 75% of your weekly
income or $250/week benefit, whichever is the lesser.
52 weeks benefit.
Excess 14 days per claim.
Parents Allowance
Pays up to 75% of non medical expenses to a limit of $1500 being for home duties etc. (provided by an authorised service). Excess 14 days per claim.
